Uplift, Inc is seeking a Full-Time Case Manager to provide Services to adults with intellectual disabilities, and/ or autism. Responsibilities of the Case Manager will include assisting in the development of the person centered plan, coordination of all necessary services, working as a team with other service providers, advocacy as well as monitoring for health and safety of the individuals. Qualified candidates will have atleast three years experience working in the field of developmental disabilities or brain injury and have atleast one year experience in case management. A bachelor's degree in social work or a related field is required. Hours and Travel vary based and case load.
