PER DIEM HOME VISITOR OPENING

SENIORSPLUS is looking for per diem Home Visitors statewide. Due to the pandemic face-to-face visits are not an option at this time, so this work would be done remotely using telephonic and/or virtual options with clients participating in home services under Long Term Care programs. A great opportunity for nurses, social workers and or other licenses healthcare professionals to supplement their income.

Responsibilities include: evaluating the effectiveness of the plan of care, assessing the participant’s health and welfare, assessing for unmet needs, and developing back-up plans and goals with the client. You will communicate with SeniorsPlus care coordinators to assist clients in receiving appropriate, effective and efficient services to achieve the maximum amount of independence possible and positively impact and improve well-being and health care outcomes for disabled children, adults and older people who are participating in at-home Long Term care programs as prescribed.

Qualifications:

Must possess a 4 year degree in the health or social services field or be a Licensed Social Worker, Registered Nurse or other licensed health professional within the State of Maine.

Must have a minimum of one year of community service.

Must have a working knowledge of the long-term care system, MaineCare and Medicare as well as Home Care Options.

Case management experience preferred.

Must have a valid Maine driver’s license.

Must have excellent clinical, problem solving, time management and prioritization/organizational skills.
