SeniorsPlus has immediate openings in their EIM Care Coordination Program for full-time salaried positions in the Belfast area. The position(s) provide support primarily through telephonic care coordination, assisting clients participating in Long Term Services and Supports to achieve maximum independence and positively impact and improve their well-being and health care outcomes through a collaborative process of assessment, planning, implementing, monitoring and coordinating with other health care professionals to coordinate the client’s plan of care. Some travel and home visits are required (currently suspended).
EDUCATION and/or EXPERIENCE
Bachelor’s degree in Health or Social Services or related field, Licensed Social Worker or eligible to obtain licensure, or other licensed health professional, within the State of Maine, with a minimum of one year community experience. Familiarity with the long term care system a plus, strong computer skills required. Ability to work independently and as part of a team.
