Job Status


The Care Coordinator is responsible for benefit eligibility and/or re-certification decisions, identification and coordination of care, education of claim program and processes, providing information, and offering referrals. This position interacts directly with the insured or their legal representative, providers, and health care professionals to provide accurate, timely, and responsive replies and resolution.

Opportunity to work from home once trained and in good standing. This is a part time position, 20-24 hours per week.

Required
Registered nurse with 5+ years' experience in a variety of medical settings. Excellent assessment skills and professional judgment. Excellent verbal and written skills. Ability to work independently with minimal supervision. Flexible with ability to handle multiple tasks simultaneously. Good telephone skills. Proficiency with MS Office Suite. Completion of the AHIP Long Term Care courses within 2 years of employment. Required to uphold the principles of compliance and fraud management as outlined by corporate and departmental policies. Supports and participates in the mandatory training on an annual or more frequent basis, as required

Preferred
5+ years' experience in a variety of medical settings, preferably in long term care or home health settings. Experience in Case Management. Previous telephonic experience.
