Easterseals NH is currently seeking an energetic, knowledgeable, flexible individual to join our team and ServiceLink Program. Through contracts with local agencies around the state, ServiceLink helps individuals access and make connections to long term services and supports, access family caregiver information and supports, explore options and understand and access Medicare and Medicaid.

In this role, you will be responsible for implementing a person centered counseling approach to understand the needs of individuals in order to assist in the development of resource options and planning for long term support services, providing guidance and referral information to callers on community resources, agencies and organizations available as possible solutions to the needs of the caller and assisting individuals in understanding and applying for State and Federal programs.

Qualifications: This position requires a Bachelor's Degree in an Interdisciplinary field, Human Services preferred and one (1) year of experience in social work, preferably in the areas of aging, disabilities, community health, nursing home or hospital discharge planning. Knowledge of Medicare/Medicaid is preferred, not required.

Compensation: Pay is based on education and experience.

Hours: This is a full-time position located in Nashua, NH.
