Essential Duties and Responsibilities:
Provide intake, assessment, treatment, inter-agency collaboration, and follow-up or referral services for caseload assigned.
Develop, monitor and modify individual service plans for clients.
Meet regularly with clients.
Assist clients in discharge planning for Medication Assisted Treatment in the community.
Maintain all clinical records in accordance with agency, program, licensing, and clinical standards.
Provide Day One’s Program Manager with timely statistics and client data necessary to fulfill contract and billing requirements.
Participate in clinical supervision, training and staff meetings as directed by Program Manager or Chief Clinical Officer.
Represent Day One professionally in the community as directed by Program Manager or Chief Clinical Officer.
Performs all other duties as assigned.

Physical Environment:
Most duties will be performed at Mountain View Correctional Facility. Some travel required to other Day One locations as well as to outside events.
Physical Effort:
Physical effort commensurate with routine office work, with extensive use of a computer, and with local travel. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
Requirements
++Requirements and Skills Needed for Position++:
- BS/BA degree in human services, counseling or related field.
- Ability to consistently work collaboratively and effectively as a team member.
- Good verbal, written and interpersonal communication skills.
- Reliable transportation and ability to travel locally.
- Ability to work flexible hours as needed to meet needs of clients.
