Aspire Behavioral Health & Counseling is seeking to fill a full-time position for a dual role as a Community Support Supervisor (CSS) and Children’s (Targeted) Case Manager (CCM) *in the *Franklin County **_area.**_ This position reports to the Clinical Director.

As a CSS, you’ll *work *within the MaineCare Section 28 program to provide services in the home, community and office setting. The *CSS *provides support and supervision to the Community Support Workers (CSWs) that work directly with children and families. Duties include working with children, families, CSWs, and mental health treatment teams in creating skill-based treatment plans for children; and completing documentation with families as stated by the MaineCare regulations.

As a CCM, you’ll work collaboratively with children (age birth-20 years) that have a mental health diagnosis, developmental delay, intellectual disability and/or chronic medical condition. Duties include working with the child, family, and their treatment team to help the child attain and achieve identified support plan goals. Will also conduct intakes and coordinate comprehensive assessments of the children’s and family’s strengths and needs; produce an Individualized Support Plan (ISP); coordinate, advocate for and develop services identified in the plan; monitor progress; and evaluate the appropriateness and effectiveness of services.

Requirements

Education and Experience:

Bachelor's degree in human services or related field and at least 2 years related experience; or Be a licensed social worker (LSW) or Licensed Social Worker, Conditional (LSX) with at least (one) year of related experience; or Be a licensed social worker (LSW) who has attained a related Master's degree; or Be a licensed professional counselor (LPC), licensed clinical professional counselor (LCPC); licensed clinical professional.

Other Job Requirements:

Must possess a current CANS (Child and Adolescent Needs and Strengths) Assessment training certificate, through the Praed Foundation, as required by the State of Maine; or ability to train for and obtain the certificate within 10 days of hire.

Must have a valid Maine Driver's license, reliable transportation, and current automobile liability insurance. Must possess a good driving record and pass a criminal and DHHS background check.

Location:

Works primarily out of the main office in Smithfield and requires travel throughout the assigned region.
